Why we need this information
We need to keep accurate records in order to adhere to the legislation on obtaining written consent for a medical treatment. In the event of an adverse reaction to your treatment, this information would be required and shared with other health care professionals to assist with any further medical intervention.
What we do with your information
Aesthetic treatments of dermal filler, Botox for Migraine and Hyperhidrosis record sheets, including computer spreadsheets will not be passed on to any third party, unless a medical emergency should occur and you are in need of further medical intervention. You will have agreed to this on your written consent form.
Patients who have had a Greater Occipital nerve block will have the option to have this information sent to their NHS GP/Headache consultant. This information will not be passed on to any third party, unless a medical emergency should occur and you are in need of further medical intervention. You will have agreed to this on your written consent form.
How we store and how long we keep your information
Your information is securely stored under lock and key and is password protected
We keep your written consent and treatment records for 5 years We will then dispose your information by paper shredding and secure deletion of computer record.

Your right to rectification - You have the right to ask us to rectify information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to object to processing - You have the the right to object to the processing of your personal data in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
